We are pleased to fund the following delegates for our 2025 conference thanks to the generous support of MiCare:

    • 2 NICU delegates per participating EPIQ site
    • 1 MGM/OB delegate per participating CPTBN site
    • 10 parent-partners – all sponsored spots already assigned

    Funded delegates will receive coverage for their registration, accommodation at Hôtel Bonaventure, meals, and transportation via flights.

    Funding statuses are determined by site investigators of EPIQ, CPTBN, and CNFUN. Please confirm your funding status with your site investigator before registering.

    PLEASE NOTE:

     

    • For reimbursements, you must retain an itemized receipt with debit/credit confirmations for all eligible reimbursements
    • Boarding passes must be retained for reimbursements
    • Travel and hotel final, itemized email receipts must be obtained for reimbursements
    • Reimbursements or refunds will be processed after the conference

    Accommodation and Reimbursement Information
    • MiCare staff will not handle hotel reservations for MiCare-funded delegates with the exception of CPBF Parent-Partners, Speakers, and CNN Abstractors’ Representative.
      • CPBF Parent-Partners and Speakers: MiCare will arrange your hotel reservations and cover accommodation costs for a two-night stay on Saturday, February 8, 2025, and Sunday, February 9, 2025.
      • CNN Abstractors’ Representative: : MiCare will arrange your hotel reservations and cover accommodation costs for a one-night stay on Monday, February 10, 2025.
      • Please specify your check-in and check-out dates when completing the registration form.
    • Exclusive Discounted Rate: As a valued conference delegate, you can access an exclusive discounted rate (plus applicable fees and taxes) at Hotel Bonaventure. This offer is valid until Wednesday, January 8, 2025. After this date, standard room rates will apply based on availability.

    • Reimbursement Details:
        • Accommodation costs will be reimbursed after the conference.
        • MiCare will reimburse delegates for a two-night stay on Saturday, February 8, 2025, and Sunday, February 9, 2025, up to a maximum rate of $219.00 + tax per night.
        • EPIQ workshop facilitators will be reimbursed for a one-night stay on Friday, February 7, 2025.
    • Cancellations and Changes: For cancellations or changes to your itinerary, please contact the hotel directly at 514-878-2332 or 1-800-267-2575. Note that full cancellation penalties apply for cancellations made within 72 hours of arrival.

     

    Travel and Reimbursement Information
    • Delegates are responsible for booking their own airline reservations.
    • Reimbursement Details:
      • Travel expenses will be reimbursed after the conference.
      • Economy Airfare and Baggage Fees: These will be reimbursed.
      • Seat Selection and Travel Insurance: These expenses are not reimbursed.
      • Additional Transportation Costs:
        • Transportation to and from your home and local airport will not be reimbursed.
        • If you stay offsite, transportation to and from the hotel will not be reimbursed.

    Meals
    • The following meals are offered and covered for funded delegates at the EPIQ Conference:
      • Saturday, February 8th: PM Snack, Dinner
      • Sunday, February 9th:  Breakfast, AM Snack, Lunch, PM Snack
      • Monday, February 10th: Breakfast, AM Snack
    • All other meals will not be reimbursed (including in-flight meals and meals at airport)
